I've been going through the Titan Quest Skill Calculators, and I know of all the left click attacks that you use for a main attack. Warfare has Onslaught. Dream has Psionic Strike. Spirit as Ternion Attack. Rune has Rune Weapon. Rogue has Calculated Strike. And Hunting has Marksmanship. None of these attacks has a recharge time so they can be used indefinitely as long as you can meet the energy needs.
But the other masteries, Earth, Storm, Nature, and Defense have no skills that you can use as a left-click attack without recharge. So if you make a character using two of those masteries, you'll have nothing special. Which means you may need to just use the basic attack. And some of the other masteries listed above in the first paragraph have the same problem if you use an attack style or weapon that cannot use the attack. For example, the melee Spellbinder, the Spear Warden, the Guardian, melee Paladin, melee Juggernaut, etc.
So what do you do if your character has no left-click attack that lacks recharge?

Just use the basic LMB one. It's not a big deal.

What if you're a spellcasting class? There aren't a lot of spells that have no recharge. Even some of the best spells that would be suitable for left click have some recharge.

Just use staff attacks inbetween spellcasting myself. And with -%recharge gear you could get close to spamming some spells if you wanted and could cope with the energy cost.

Batter with 50 to 80% recharge is good choice
You miss Lightning dash and Ice shard in Storm. Ice shard is one of the most powerful LMB in TQ

Lightning Dash isn't really suitable for a combat attack. That's why I didn't list it.

Ternion is an LMB attack? I use it as an RMB attack like an explosive shotgun if/when mobs close in, but I don't use it as my primary LMB...

If I have Ternion I put it on the LMB on the second weapon swap and simply change over if I feel I need it.

Ternion "can" be a LMB , if you got a good mana recovery ::)

Ah well I have 5 configurable buttons on my mouse so I can bind a lot to it.

I read that if you max Barrage and get two extra skill levels, Flame Surge can be spammed and has no cooldown. Something to consider for casters who have Earth as a mastery. Though I would think the Elementalist would rather use Ice Shard for more damage and longer ranger.
